    Im Oshani himasha Munaweera, from Matale, My dad have applied for the “CEYLINCO CHILDREN HEALTH POLICIES PLUS”. ane the policy number is “AN0010PH000133”. Unfortunately my dad have lost the documents, and i have only the card issued by Ceylinco, what am i to to with the insurance now?
